INTRODUCTION 1. Agreement to These Terms These Terms of Service ("Terms") constitute a legally binding agreement between you ("Client," "Donor," or "you") and Abdullah Lab Services INC ("Abdullah Lab Services," "we," "us," or "our"), a Pennsylvania corporation operating in Philadelphia, Pennsylvania. By scheduling an appointment, submitting a booking request, using our website, or receiving any service from Abdullah Lab Services INC, you acknowledge that you have read, understood, and agree to be bound by these Terms, as well as our Privacy Policy, which is incorporated herein by reference. If you are accessing our services on behalf of an employer, staffing agency, school, or other organization, you represent and warrant that you have authority to bind that organization to these Terms, and references to "you" include that organization. If you do not agree to these Terms, do not use our services or website.
Services We Provide
Abdullah Lab Services INC provides the following occupational health, compliance, and safety services throughout Greater Philadelphia and surrounding counties: • DOT Drug Testing Federally certified specimen collection for transportation companies and safety-regulated industries, conducted in compliance with 49 CFR Part 40 • Non-DOT Drug Testing Flexible drug screening for employers, schools, and organizations not subject to DOT requirements • Non-DOT Breath Alcohol Testing Breath and alcohol testing for private employers, schools, and organizations • DNA Testing Specimen collection and result coordination for paternity, relationship, and personal identification testing • Fingerprinting and Background Checks Ink fingerprinting and comprehensive consumer background screening for employment and licensing purposes • CPR and First Aid Training Hands-on CPR/AED and First Aid training for individuals, employers, and small groups • Mobile Notary Services Pennsylvania-commissioned notary public services for document notarization throughout Greater Philadelphia, provided by The Abdullah Group LLC Services are provided at our Philadelphia-area location and, for certain services, at client or donor locations by arrangement. Abdullah Lab Services INC reserves the right to modify, add, or discontinue any service at any time.
Scheduling, Appointments, and Eligibility
3.1 How to Schedule Services may be scheduled through our online booking system at abdullahlabservices.com or by calling (866) 251-3706. Appointment availability is subject to change. Confirmation of your appointment will be provided via email or phone. 3.2 What to Bring All clients and donors must present a valid, government-issued photo ID at the time of service. Acceptable forms include a driver's license, state-issued ID, passport, or military ID. Services cannot be performed without valid identification. For drug testing, avoid eating or drinking for 30 minutes before your collection appointment. For DNA testing, follow any pre-test instructions provided at the time of booking. Contact us at (866) 251-3706 with specific questions. 3.3 Eligibility By scheduling services, you represent that: • You are 18 years of age or older, OR you are a parent or legal guardian scheduling on behalf of a minor with proper authorization • The information you provide is accurate and complete • You are authorized to schedule the service (for employer or agency accounts) • You will comply with all collection procedures and instructions provided by our staff
Cancellation, No-Show, and Refund Policy
4.1 Cancellations and Rescheduling Appointments may be cancelled or rescheduled without penalty if notice is provided at least 24 hours before the scheduled service time. To cancel or reschedule, contact us at hello@abdullahlabservices.com or (866) 251-3706. 4.2 Late Cancellations and No-Shows Cancellations made less than 24 hours before a scheduled appointment, or failure to appear at a scheduled appointment (no-show), may result in a cancellation fee equal to the full cost of the scheduled service. This fee covers staff time, reserved laboratory resources, and administrative costs. 4.3 Refunds Refunds will be issued in the following circumstances: • Abdullah Lab Services INC is unable to perform the scheduled service due to an error on our part or circumstances within our control • A duplicate payment was made in error Refunds are not issued for: • Completed services where a specimen has been collected and submitted for laboratory analysis • No-shows or cancellations made less than 24 hours in advance • Negative or unfavorable test results • Employer or agency decisions made based on test results All refund requests must be submitted within 7 days of the service date by emailing hello@abdullahlabservices.com with your name, date of service, and reason for the request. Approved refunds will be processed within 5-10 business days.
Payment Terms
Abdullah Lab Services INC accepts credit cards and electronic payment methods for individual client services. For employers and staffing agencies, we offer invoice billing with agreed-upon payment terms. Contact us at hello@abdullahlabservices.com to arrange bulk service pricing or to set up a billing account. Payment is due at the time of service for individual clients unless otherwise agreed in writing. For employer accounts, payment is due according to the terms of your service agreement. Abdullah Lab Services INC reserves the right to withhold services or results in the event of non-payment. All fees are quoted in U.S. dollars. Prices are subject to change with reasonable notice. Your confirmed booking price will not change after confirmation.
Confidentiality and Test Results
6.1 Confidentiality Standards All test results and personal information are protected under strict confidentiality standards, including HIPAA compliance for health-related data. Abdullah Lab Services INC maintains confidentiality protocols for all specimen and test result information in accordance with applicable federal and state law. 6.2 Who Receives Results Results are released only to the parties authorized on your testing consent form or custody and control form at the time of service. For DOT testing, results are reported through the MRO process as required by 49 CFR Part 40. For employer-ordered Non-DOT testing, results are reported to the employer or designated employer representative (DER) as specified on the test order. We do not release results to any unauthorized party. 6.3 Collection Site vs. Laboratory Responsibility Abdullah Lab Services INC is a certified specimen collection site. We are responsible for proper specimen collection, chain-of-custody documentation, and accurate submission of specimens to the testing laboratory. Laboratory analysis is performed by independent, SAMHSA-certified laboratories. Abdullah Lab Services INC is not responsible for errors arising from laboratory analysis, which are the responsibility of the testing laboratory.
DOT Testing Compliance and Donor Rights
All DOT drug and alcohol testing performed by Abdullah Lab Services INC is conducted in compliance with U.S. Department of Transportation regulations, 49 CFR Part 40. As a DOT-certified specimen collection site, we follow all required chain-of-custody procedures. 7.1 MRO Review Rights Donors who receive a verified positive, adulterated, or substituted DOT drug test result have the right to request review by a Medical Review Officer (MRO). The MRO will contact the donor before reporting a verified positive result to the employer. Donors should contact the MRO identified on their Federal Drug Testing Custody and Control Form (CCF) to exercise this right. 7.2 Split Specimen Rights Under 49 CFR Part 40, donors have the right to request that the split specimen (Bottle B) be tested at a second SAMHSA-certified laboratory within 72 hours of being notified of a verified positive result. This request must be made to the MRO. Background Check Services and FCRA Compliance
When providing background check services, Abdullah Lab Services INC may act as a consumer reporting agency as defined by the Fair Credit Reporting Act (FCRA), 15 U.S.C. § 1681 et seq. 8.1 Employer Obligations Employers and organizations ordering background checks through Abdullah Lab Services INC are solely responsible for: • Providing a clear, standalone written disclosure to the individual that a consumer report may be obtained for employment purposes • Obtaining the individual's written authorization before ordering a background check • Providing required pre-adverse and adverse action notices if any action is taken based on report contents • Complying with all applicable federal and state background check and consumer protection laws Abdullah Lab Services INC is not liable for an employer's failure to comply with FCRA requirements. 8.2 Individual Rights Individuals have the rights described in our Privacy Policy, Section 8, including the right to dispute inaccurate information and the right to receive a copy of any consumer report used against them.
CPR and First Aid Training
CPR and First Aid training provided by Abdullah Lab Services INC is intended to teach general skills for emergency response. By participating in training, you acknowledge: • Training completion does not guarantee competence in every emergency situation • CPR/AED skills require regular practice and renewal to remain effective • Training is not a substitute for professional medical care always call 911 in a medical emergency • Certifications issued upon completion are valid for the period specified on the certificate and must be renewed before expiration Abdullah Lab Services INC is not liable for outcomes resulting from the application or non-application of CPR or First Aid techniques in real emergency situations.

ABDULLAH LAB SERVICES INC IS A SPECIMEN COLLECTION SITE AND TESTING SERVICES PROVIDER. WE DO NOT PROVIDE MEDICAL ADVICE, MEDICAL DIAGNOSIS, OR MEDICAL TREATMENT OF ANY KIND. Test results provided by Abdullah Lab Services INC reflect laboratory analysis of collected specimens and should not be interpreted as a medical opinion, diagnosis, or treatment recommendation. If you have questions about your health, the interpretation of a test result, or the medical implications of any finding, consult a licensed healthcare provider. Nothing on our website, in our communications, or in our service documentation constitutes medical advice.
Limitation of Liability
11.1 Our Responsibility Abdullah Lab Services INC is responsible for proper specimen collection, chain-of-custody procedures, and accurate reporting of results as provided by the certified testing laboratory. 11.2 What We Are Not Liable For To the fullest extent permitted by applicable law, Abdullah Lab Services INC shall not be liable for: • Errors in laboratory analysis, which are the responsibility of the testing laboratory • Employment decisions, terminations, or legal consequences resulting from test results • Outcomes of legal proceedings or regulatory actions in which test results are used as evidence • Delays in result reporting caused by laboratory processing time or circumstances outside our control • Any indirect, incidental, consequential, or punitive damages arising from the use of our services 11.3 Cap on Liability In no event shall Abdullah Lab Services INC's total liability to any client or donor exceed the total amount actually paid by that client for the specific service giving rise to the claim. 11.4 Exceptions This limitation of liability does not apply to claims arising from Abdullah Lab Services INC's gross negligence or intentional misconduct, or to any liability that cannot be limited under applicable law.
Indemnification
You agree to indemnify, defend, and hold harmless Abdullah Lab Services INC, its owners, employees, agents, and contractors from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorneys' fees) arising out of or related to: • Your use of our services or website • Your breach of these Terms • Your violation of any applicable law or regulation • For employer and agency clients: your failure to comply with FCRA requirements, DOT regulations, or other laws governing the use of testing services • Any inaccurate information you provide to us in connection with scheduling or receiving services
Consent to Services and Agreement
By scheduling an appointment, using our online booking system, or receiving services from Abdullah Lab Services INC, you acknowledge and agree that: • You have read and agree to these Terms of Service and our Privacy Policy • You consent to the collection of specimens, personal information, or biometric data required to perform the requested service • You authorize Abdullah Lab Services INC to release results to the parties identified on your consent or custody form • You understand that Abdullah Lab Services INC does not provide medical advice or diagnosis • You have provided accurate and truthful information in connection with your appointment For DOT-regulated testing, consent is formally documented on the Federal Drug Testing Custody and Control Form (CCF) as required by 49 CFR Part 40. Donor refusal to sign the CCF or to cooperate with collection procedures constitutes a refusal to test under DOT regulations. For employer and agency accounts: by ordering services on behalf of a donor, you represent that you have obtained all legally required authorizations from the individual donor prior to ordering testing

Force Majeure
Abdullah Lab Services INC will make every reasonable effort to fulfill scheduled appointments and deliver services as agreed. However, we shall not be liable for any delay or failure to perform services caused by circumstances beyond our reasonable control, including but not limited to acts of God, natural disasters, severe weather, fire, public health emergencies, pandemic, government orders or regulations, utility failures, equipment failures, or civil unrest. In such cases, we will contact you as soon as practicable and work with you to reschedule your appointment at no additional charge. No cancellation fee will be assessed when a service cannot be completed due to a force majeure event on our end.
Website Use and Intellectual Property
The content of abdullahlabservices.com, including text, images, logos, and design elements, is owned by or licensed to Abdullah Lab Services INC and is protected by applicable intellectual property laws. You may not reproduce, distribute, or use our content for commercial purposes without our prior written consent. You may use our website for the purpose of learning about our services, scheduling appointments, and communicating with us. You may not use our website to transmit harmful, fraudulent, or unlawful content, or to interfere with the operation of our website or services.
Governing Law and Dispute Resolution
16.1 Governing Law These Terms of Service are governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania, without regard to its conflict of law principles. 16.2 Jurisdiction Any dispute arising out of or relating to these Terms, our Privacy Policy, or your use of Abdullah Lab Services INC's services shall be subject to the exclusive jurisdiction of the state and federal courts located in Philadelphia County, Pennsylvania. By using our services, you consent to personal jurisdiction in Philadelphia County, Pennsylvania. 16.3 Informal Resolution Before initiating any legal proceeding, you agree to first contact Abdullah Lab Services INC at hello@abdullahlabservices.com to attempt to resolve the dispute informally. We will make a good-faith effort to resolve your concern within 30 days.
General Provisions
17.1 Entire Agreement These Terms of Service, together with our Privacy Policy and any service-specific agreements, constitute the entire agreement between you and Abdullah Lab Services INC regarding your use of our services and supersede any prior agreements or understandings. 17.2 Severability If any provision of these Terms is found to be invalid or unenforceable by a court of competent jurisdiction, the remaining provisions will continue in full force and effect. 17.3 Waiver Our failure to enforce any provision of these Terms shall not constitute a waiver of our right to enforce that provision in the future. 17.4 No Agency Nothing in these Terms creates a partnership, joint venture, employment relationship, or agency relationship between you and Abdullah Lab Services INC. 17.5 Updates to These Terms We may update these Terms from time to time to reflect changes in our services, legal requirements, or business practices. When we make material changes, we will update the "Last Updated" date at the top of this document. Your continued use of our services after any update constitutes acceptance of the revised Terms.
